Referral Process
How to Make a Placement
Our streamlined process ensures placements are assessed and confirmed efficiently.

Initial Enquiry
Contact us with the young person's basic details and placement requirements. We will confirm availability promptly.
- 02.
Referral Assessment
Provide the referral pack including Pathway Plan, risk assessment, and any relevant history. We assess suitability.
- 03.
Pre-Placement Visit
Where possible, we arrange a visit to the property so the young person can see the accommodation before moving in.
- 04.
Placement Agreement
Once agreed, we formalise the placement with a clear agreement covering costs, support hours, and notice periods.
Before Placement
Information We Need
- Full name, date of birth, and legal status
- Current Pathway Plan
- Risk assessment (including any known risks)
- Personal Advisor contact details
- Placement funding confirmation
- Any specific support needs or preferences
