Referral Process

How to Make a Placement

Our streamlined process ensures placements are assessed and confirmed efficiently.

  1. 01.

    Initial Enquiry

    Contact us with the young person's basic details and placement requirements. We will confirm availability promptly.

  2. 02.

    Referral Assessment

    Provide the referral pack including Pathway Plan, risk assessment, and any relevant history. We assess suitability.

  3. 03.

    Pre-Placement Visit

    Where possible, we arrange a visit to the property so the young person can see the accommodation before moving in.

  4. 04.

    Placement Agreement

    Once agreed, we formalise the placement with a clear agreement covering costs, support hours, and notice periods.

  • Full name, date of birth, and legal status
  • Current Pathway Plan
  • Risk assessment (including any known risks)
  • Personal Advisor contact details
  • Placement funding confirmation
  • Any specific support needs or preferences
